If you are the subject of an emergency direction not to board an aircraft under the Passenger Protect Program, you may apply in writing to the Office of Reconsideration to request a reconsideration of your specification by completing the Application to the Office of Reconsideration under the Passenger Protect Program form and submitting it to initiate the reconsideration process.
How do I complete the form?
To ensure that your application is properly considered, it is important that you complete the form carefully. Please include all details pertaining to your situation. Use the following guidelines:
Section A: Applicant Information
- Print your personal information including your name, address of permanent residence and telephone number.
Section B: Grounds for Seeking Reconsideration
- Describe the reason why you are applying to the Office of Reconsideration.
- Explain clearly why you believe you should not be specified. Do not limit yourself to the space provided on the form; if you need more space to provide a full explanation, attach a separate piece of paper.
- If you have a copy of the emergency direction not to board an aircraft under the Passenger Protect Program, attach it to your application.
Section C: Privacy Act Statement and Consent
- Read, sign and date this section.
Certified ID
- Your application must include a certified photocopy of at least two pieces of government-issued identification that support your identity (e.g. documents issued by federal or provincial/territorial authorities). Please do not send us your original documents. Get more information about acceptable identification and how to get photocopies certified.
